The 2026 Medi-Cal Billing Guide for California Practices

Medi-Cal covers roughly a third of Californians, which makes it the single most important payer relationship for most practices in the state. It is also the payer where small process errors cost the most money.

The first place practices lose revenue is enrollment. A provider who is active with Medi-Cal fee-for-service is not automatically contracted with the managed care plans that actually administer most member benefits. Verify both before the first visit.

The second is timely filing. Medi-Cal timely filing windows are shorter than most commercial payers, and a claim that sits in a rejection queue for three weeks may already be unrecoverable. Rejections need a daily owner, not a weekly one.

Finally, document medical necessity in the language the plan's reviewers use. Appeals succeed when the note answers the specific criteria in the plan's policy, not when it simply restates the diagnosis.
